ZOOM REGISTRATION FORM. Please fill out this form to the best of your ability and submit to your respective Judge’s Chambers two weeks in advance of your Zoom hearing or trial. Should you require more space, attach a supplement to this form. If you have any questions, please refer to the Zoom Guidelines on the “For Attorneys” tab of the Court’s website and/or consult your Judge’s Chambers. Parties are responsible for providing participant e-mail addresses to the Court, as well as a brief description of the role that each participant plays in the case, including any witnesses, experts or other interested, participating parties. Parties must also provide a list of phone numbers at which all participating parties can be readily reached. Finally, a brief description of the type of technology (e.g., Mac, PC, iPad) that each party will be using will assist Court personnel in quickly contacting and providing appropriate technical assistance to any party experiencing technical issues during the Zoom proceeding. www.FormsWorkflow.com
