SC-517 - DEMAND FOR TRIAL AND INSTRUCTIONS (SMALL CLAIMS). This form is used in the Wisconsin Circuit Court when a party wishes to request a trial before a circuit court judge following a court commissioner’s decision in a small claims case. Either the plaintiff or defendant may file this form, depending on who disagrees with the commissioner’s ruling.
