Description

AOC-CV-608 - APPLICATION, SUMMONS AND ORDER TO SHOW CAUSE - CHILD SUPPORT. This form is used in the State of North Carolina is a legal document filed in district court to initiate proceedings for child support. This form is typically used when the paternity of a child has been acknowledged by the defendant and affirmed by the mother, but the defendant has failed to sign a voluntary support agreement. The applicant, often the custodial parent or a representative of child support enforcement, requests the court to summon the noncustodial parent and order them to appear and explain why a child support order should not be issued. The form includes the names and addresses of the parties involved, the defendant’s employer, and any need for court interpreters. Once filed, a judicial official issues a summons and sets a court date where the defendant must appear to respond. Failure to attend the hearing may result in the court entering a support order by default. The form also includes a section for law enforcement to complete a return of service, verifying how and when the defendant was served with the summons. G.S. 110-132(b). www.FormsWorkflow.com
