This form is used in criminal cases involving drug-related offenses to document a defendant's agreement to participate in a court-approved treatment program instead of proceeding to sentencing or probation. This form outlines the conditions the defendant must accept in order to be eligible for deferred entry of judgment (DEOJ), including undergoing a confidential substance abuse and mental health evaluation, complying with a personalized treatment plan, attending court and treatment sessions, submitting to random drug testing, and abstaining from drugs, alcohol, and weapons.
