IC8 - RESPONSE TO PETITION FOR AN IMMEDIATE HEARING UNDER SECTION 19(b) OF THE ACT. This form is used in Illinois workers’ compensation cases when an employer or their representative replies to an employee’s request for an expedited hearing. Under the law, the employer must file this response within 15 days of receiving the petition. The form allows the employer to state whether they agree or disagree with the petitioner’s claims regarding key facts such as the date, time, and location of the accident; the description and nature of the injury; notice of the accident; refusal to pay benefits; medical treatment and providers; disputed medical bills; disability status; and any efforts to resolve the dispute. The reverse side of the form is used to explain each disagreement in detail.
