DFPI–EL 326 - APPLICATION FOR AUTHORIZATION TO CHANGE OWNERSHIP. This form is used for an entity operating under the Escrow Law in California. This application is to be filed for stock and ownership change of 10% or more and must be accompanied by a filing fee of $625.00 and an investigation fee of $100.00 (Section 17213 and Section 17207, Financial Code). The application requires details about the corporation’s principal office, licensed branch locations, and new officers, directors, stockholders, and managers following the ownership change. Applicants must submit supporting documentation, including identity verification, fingerprint records, financial statements, affidavits, and a disclosure authorization form. The proposed escrow agent manager must meet the qualifications outlined in Financial Code Section 17200.8. Both the buyer and seller must sign the form, and if it is executed outside California, it must be notarized.
