LBF-P - DEBTOR’S AFFIDAVIT REQUESTING DISCHARGE. This form is used in the United States Bankruptcy Court for the District of Maryland by individuals who have completed their Chapter 13 bankruptcy repayment plan and are requesting a discharge of their remaining eligible debts. The affidavit serves as a sworn statement in which the debtor certifies that they meet all the legal requirements for discharge under the Bankruptcy Code.
