NOTICE OF SUPERVISED ADMINISTRATION. This form is issued by the Allen Superior Court, Probate Division, in the State of Indiana, to formally inform interested parties that a personal representative has been appointed to administer the estate of a deceased individual under court supervision. This notice identifies the appointed personal representative, the decedent, and the date of death. It also serves as an official alert to creditors and other claimants that any claims against the estate—regardless of whether they are currently due—must be filed with the Clerk of the Court within three months from the first publication date of the notice or within nine months after the decedent's death, whichever is earlier. Failure to file claims within this period will result in those claims being permanently barred. The notice is issued in accordance with Indiana Code 29-1-7-7(c) and (d), and must be served through Indiana’s E-filing System or by first-class mail to each heir, devisee, legatee, and known creditor.
