Guardians Oath

GUARDIAN’S OATH. This form is used in the Allen Superior Court, Probate Division, in the State of Indiana, is a sworn statement completed by an individual who has been appointed as a guardian over the person, the estate, or both, of a minor or adult. By signing this oath, the appointed guardian formally accepts their role and affirms, under penalty of perjury, that they will faithfully perform their duties in accordance with Indiana law.
