SFACC-002 - REQUEST FOR JURY FEE DEPOSIT REFUND. This form is used in the Superior Court of California, County of San Francisco, to request a refund of a previously posted $150 jury fee deposit. Refunds are available only for deposits made after January 1, 1999, and before June 27, 2012. The request must be submitted within 20 business days of a jury waiver, settlement, dismissal, or qualifying continuance.
