SDSC CIV-365 - DECLARATION CONCERNING WAIVED COURT FEES. This form is used in San Diego Superior Court when a party seeks to enter satisfaction of judgment in a case where court fees were previously waived. The form requires the declarant to state, under penalty of perjury, whether all waived court fees and costs have been repaid to the court.
