JD-FM-295 - REQUEST FOR REMOTE TESTIMONY. This is a Connecticut Judicial Branch Superior Court form used by a party in a family court case who is protected by a restraining order, protective order, or standing criminal protective order and wishes to testify without appearing in the presence of the other party. The form allows the applicant to request either remote testimony from home or another location, or testimony from within the courthouse but outside the other party’s presence.
