JD-FM-249 - CERTIFICATION OF WAIVER OF SERVICE OF PROCESS - DIVORCE, LEGAL SEPARATION, ANNULMENT. This is a Connecticut Judicial Branch form that allows a defendant in a divorce, legal separation, annulment, or civil union dissolution case to voluntarily waive formal service of process. Instead of being personally served by a state marshal or other proper officer, the defendant may acknowledge receipt of the Summons, Complaint, and Notice of Automatic Orders and consent to proceed with the case.
