JD-FM-138 - AFFIDAVIT - RELIEF FROM ABUSE. This is a Connecticut Judicial Branch Superior Court form used to provide a sworn statement in support of an Application for Relief From Abuse (JD-FM-137). The affidavit allows an applicant to describe the incidents and circumstances that form the basis for requesting a restraining order, including what happened, when and where the events occurred, and who was present. Applicants must also indicate whether they are seeking temporary custody of minor children and, if so, submit an Affidavit Concerning Children (JD-FM-164). The affidavit must be completed under oath and signed before a court clerk, notary public, commissioner of the Superior Court, or authorized attorney.
