VN015 - CERTIFICATE RE: DISQUALIFICATION OF JUDGE (CCP 170.6). This form is used in the Superior Court of California, County of Ventura, when a party or their attorney believes that the assigned judge is prejudiced against their interests and that a fair and impartial trial cannot take place before that judge.
