VN274 - MEDIATOR’S NOTICE OF ACCEPTANCE OR RECUSAL.This form is used in the Superior Court of California, County of Ventura, to formally document whether a court-assigned mediator agrees to serve in a particular case or declines the assignment. This form must be completed and served on the court and all parties within ten days of receiving the Notice of Assignment of Mediator. If the mediator accepts the assignment, they must indicate whether the mediation has been scheduled and provide the date, time, and location if applicable.
