Description
JGM401 - EXEMPTION FORM. This is a Minnesota District Court form used by a debtor to claim that some or all money frozen by a bank levy, garnishment, or other collection action is legally protected from creditors. This form allows the debtor to identify exempt funds, such as wages, government benefits, child support, retirement payments, insurance proceeds, and other protected income. The form requires the debtor to explain why the funds are exempt, list applicable benefit programs, and attach supporting documents like bank statements or benefit notices. Completed copies must be sent to the creditor (or creditor’s attorney) and the bank or third party holding the funds. Minn.
