Description
Form 5-7 - ORDER OF FILIATION. This form is the final order that adjudges and declares paternity in an Article 5-B proceeding under FCA § 542. Issued after a paternity petition is filed and heard, it notes appearances/admissions or a default, and then names the father and child. By consent, the order may also change the child’s legal name, with space for additional directives. Standard notices explain how to vacate a default, and set deadlines to appeal a judge’s order (30 days from receipt/service or 35 days from clerk mailing) or file written objections to a support-magistrate order (30/35 days). www.FormsWorkflow.com
