Form TPR-15 - PETITION FOR MODIFICATION OF ORDER OF DISPOSITION TO RESTORE PARENTAL RIGHTS. This is used in the New York Family Court to request the restoration of parental rights after termination. This petition may be filed by the parent whose rights were terminated, an authorized agency, the child’s attorney, or the guardian or custodian. It must demonstrate that restoring parental rights serves the best interests of the child, who must be at least 14 years old, still under court jurisdiction, and not yet adopted.
