UJS-157 - PETITION TO TERMINATE GUARDIANSHIP ON DEATH OF PROTECTED PERSON. This form is used by a guardian to report a protected person’s death and request termination of the guardianship. The sworn filing lists the original appointment date and the date/time of death, attaches a certified death certificate (SDCL 29A-1-107(2)), and—if not waived—includes a Guardian’s Final Report for court approval (SDCL 29A-5-403(4)).
