IC33 - DEDIMUS POTESTATEM. This form is used when a necessary witness in a workers' compensation case is unable to attend a hearing in person. This form appoints an individual—usually a legal professional or notary—to act on behalf of the Commission to take the deposition of such witnesses under oath. It authorizes the appointed person to administer both oral and written questions posed by either the petitioner (employee) or the respondent (employer) at a specific time and place.
