LBF-Q - STATEMENT UNDER PENALTY OF PERJURY CONCERNING PAYMENT ADVICES DUE PURSUANT TO 11 U.S.C. § 521(a)(1)(B)(iv). This form is used in Chapter 13 bankruptcy cases filed in the United States Bankruptcy Court for the District of Maryland. This form is required when a debtor cannot submit copies of payment advices (i.e., pay stubs or other proof of earnings) received from an employer during the 60 days prior to filing the bankruptcy petition.
