LBF-Q - STATEMENT UNDER PENALTY OF PERJURY CONCERNING PAYMENT ADVICES DUE PURSUANT TO 11 U.S.C. § 521(a)(1)(B)(iv). This form is used in Chapter 13 bankruptcy cases filed in the United States Bankruptcy Court for the District of Maryland. This form is required when a debtor cannot submit copies of payment advices (i.e., pay stubs or other proof of earnings) received from an employer during the 60 days prior to filing the bankruptcy petition. The debtor must complete this statement to explain why such documentation is not being provided. The form offers several options for explanation, such as the debtor not being employed during the relevant time frame, being employed but not receiving any payment advices, being self-employed and thus not receiving traditional pay statements, or another specific reason which must be explained.
