NOTICE OF UNSUPERVISED ADMINISTRATION TO BE MAILED TO A CREDITOR. This form is issued by the Allen Superior Court, Probate Division, in the State of Indiana to inform interested parties, specifically creditors, that a personal representative has been appointed to administer the estate of a deceased person without court supervision. This notice identifies the personal representative, the decedent, and the date of death. It advises all individuals or entities with claims against the estate—regardless of whether the claims are currently due—that they must file their claims with the Clerk of the Court within three months from the date of the first publication of the notice or within nine months after the decedent's death, whichever is earlier. Failure to file within these deadlines results in the claims being forever barred.
