SFCIV-014 - AMENDMENT – FICTIOUS .This form is used in the Superior Court of California, County of San Francisco, to amend a complaint or cross-complaint when a defendant was originally identified by a fictitious name (such as “Doe” or “Roe”) under Code of Civil Procedure § 474. Once the plaintiff or cross-complainant discovers the defendant’s true legal name, this form requests that the pleading be amended to reflect the correct name wherever it appears.
