SFACC-003 - REQUEST FOR REFUND. This form is used in the Superior Court of California, County of San Francisco, to request reimbursement of court fees or other payments made to the court. The claimant must provide identifying information, the amount requested, transaction dates, fee tag or receipt numbers, and the related case number or title. A detailed explanation of the reason for the refund is required for consideration. The form must be signed under penalty of perjury and submitted to the court’s Accounting Department at 400 McAllister Street, Room 205.
