Mediators Selection Form {CIV-005}

SDSC CIV-005 - MEDIATOR SELECTION FORM. This form is used in San Diego Superior Court when a civil case has been referred to mediation. It identifies the assigned judge, department, case management conference date, and the number of days allowed to complete mediation. The parties must jointly select a primary and alternate mediator and certify that the selection was agreed upon by all parties or counsel. If this case was not referred to mediation at the Case Management Conference, a Stipulation to Use Alternative Dispute Resolution (ADR) form (SDSC Form #CIV-359) or other written stipulation to mediate must be filed with the court prior to the assignment of a mediator.
