ACG-1 - REPORT OF COMPENSATION PAID. This form is issued by the New Jersey Department of Labor and Workforce Development, Division of Finance and Accounting, for employers to report workers’ compensation payments made during the period July 1, 2024 through June 30, 2025. Required under R.S. 34:15-94c.(1), this form must be completed and submitted no later than July 30, 2025. Employers must provide their FEIN, REG number, the total compensation paid, and the portions attributable to medical and funeral expenses as well as payments made under “disputed claims” settlements pursuant to R.S. 34:15-20. After subtracting excluded amounts, employers certify the final figure and acknowledge that late filings may incur a $100 penalty.
