LP/UNA 128 - REGISTRATION OF UNINCORPORATED NONPROFIT ASSOCIATION PURSUANT TO CALIFORNIA CORPORATIONS CODE SECTION 21300. This form is used to allow for the registration of various aspects of an association, such as its name, insignia, alteration, or cancellation. Association includes any lodge, order, beneficial association, fraternal or beneficial society, historical, military, or veterans organization, labor union, foundation, or federation, or any other society, organization, or association, or degree, branch, subordinate lodge, or auxiliary thereof. The form requires information about the association's name, address, nature of alteration (if any), and description of any insignia. Additionally, it must be signed by a chief officer of the association and may optionally include the signature of an additional officer.
