JDF 1043 - WAIVER OF SERVICE (ANNULMENT). This form is used in Colorado District Courts for family cases. It allows the respondent to waive formal service of the Summons, Petition, and associated documents. By signing the waiver, the respondent acknowledges receipt of these documents and agrees not to contest the manner in which they were served. However, the respondent retains the right to disagree with the facts or requests presented in the Petition and reserves the right to receive notices of court proceedings and to respond and appear in person.
