CCP-0240 - VECHS WAIVER AGREEMENT AND STATEMENT VOLUNTEER & EMPLOYEE CRIMINAL HISTORY SYSTEM (VECHS). Pursuant to the National Child Protection Act of 1993, as amended this form must be completed and signed by every current or prospective employee, volunteer, and contractor/vendor, for whom criminal history records are requested by the qualified entity under these laws. This form is used to authorize a requesting agency to submit an individual's fingerprints and relevant forms to the Illinois State Police and national criminal history records. This authorization allows the agency to obtain any national criminal history record pertaining to the individual from the FBI, as per 28 CFR Sections 16.30-16.34. By signing this waiver agreement, the individual authorizes the dissemination of their national criminal history record to the qualified entity with which they are employed or seeking to serve as a volunteer, in accordance with the National Child Protection Act of 1993, as amended. The individual acknowledges that, until the criminal history background check is completed, the agency may choose to deny them unsupervised access to children, the elderly, or individuals with disabilities. They also understand that they are entitled to receive a copy of the criminal history background report upon request, and they have the right to challenge the accuracy and completeness of any information contained in the report. The individual may request a prompt determination regarding the validity of their challenge before the agency makes a final decision regarding their status as an employee, volunteer, contractor, or subcontractor. www.FormsWorkflow.com
