QUITCLAIM DEED. This form is used in the Lake County Clerk's Office in Illinois to transfer ownership of real estate from the grantor(s) to the grantee(s). The form includes spaces to enter the names and addresses of the grantor(s) and grantee(s), the amount of consideration, and the form of ownership (e.g., sole ownership, joint tenancy, tenancy in common, tenancy by the entirety). The legal description of the property is provided, including the Permanent Index Number(s) (P.I.N.) and the property address. There is also an option to strike the release and waiver of homestead rights if the grantor is also the grantee. The form includes spaces for the grantor(s) to sign and print their names. A notary public then acknowledges the signatures of the grantor(s) and affixes their seal. The form also includes a space for the name and address of the preparer, as well as a space for the signature and date of the buyer, seller, or representative.
