AGREEMENT FOR VISITATION BY NATURAL PARENTS A visitation agreement was entered into on , , between and , husband and wife, residing at , herein referred to as adopting parents; and , residing at , herein referred to as natural mother; and , residing at , herein referred to as natural father. The above named parties agree as follows: 1. The referenced adopting parents hereby agree to formally adopt , the child of the above - named natural mother and natural father. 2. The natural mother and natural father hereby consent to the formal adoption of their child, , by the adopting parents. 3. The parties to the agreement concur that it is in the best interest s of the child, , that contact be maintained with the natural mother and natural father.
