RI-PR064 - PETITION FOR EVALUATION. This is a Riverside County Superior Court form used to request a court-ordered mental health evaluation. This petition is filed when there is reason to believe that an individual, as a result of a mental disorder, may be a danger to self, others, or gravely disabled and has refused voluntary evaluation. The form provides the court with identifying information about the individual and the factual basis supporting the request.
