MEX-175 - EXHIBIT COVER SHEET – ELECTRONIC MEDIA. This is a Mendocino County Superior Court form used when submitting electronic media as an exhibit for a court hearing. The form identifies the submitting party or representative, the type of electronic media being provided—such as a flash drive, CD, DVD, or other format—and the hearing date, time, and department where the exhibit will be presented.
