ANNUAL REPORT ON WARD. This is a DeKalb County, Illinois Probate Division form used by guardians to provide the court with an annual update on the condition and care of a disabled adult ward. The form gathers information about the ward’s physical, mental, and social condition, living arrangements, medical care, services received, and the guardian’s activities on the ward’s behalf. It also asks the guardian to report on the appropriateness of the current placement and whether continued guardianship is recommended.
