CH-820 - PROHIBITED ITEMS FINDING AND ORDERS (CIVIL HARASSMENT PREVENTION). This form is used to document the court’s findings regarding a restrained person’s possession, surrender, and compliance with orders related to firearms, firearm parts, and ammunition. Attached to Form CH-110, CH-116, or another restraining-order document, the form identifies prohibited items, lists their descriptions and locations, and records proof-of-compliance dates. It also schedules a mandatory review hearing at which the restrained person must show that all prohibited items were properly turned in, sold, or stored.
