PC-681 - AGREEMENT OF ADOPTION. This is a confidential Connecticut Probate Court form used to formally document an adoption agreement between the person authorized to give a minor in adoption and the adopting parent or parents. This form is used when a child is legally free for adoption under Connecticut General Statutes section 45a-724 and must be signed by all required parties, including the parent, guardian, or statutory parent giving the child in adoption and the adopting parent(s). If the minor is 12 years of age or older, the child’s written consent is also required.
