BMV 4501 - REGISTRATION APPLICATION OF PUBLIC OWNED VEHICLE. This form is used to register motor vehicles that are owned and operated by public entities, such as state, county, city, village, township, or federal government agencies. This no-fee registration allows these governmental bodies to obtain license plates for official vehicles without incurring standard registration costs, provided the vehicles are used solely for official purposes and not for private, commercial, or for-profit activities. The form must be fully completed and include essential documentation such as the original certificate of title (or bill of sale/certificate of origin for qualifying trailers), and in certain counties, a valid emissions test certificate. It also provides options to request new plates, transfer plates, replace lost or damaged plates, or obtain duplicate registrations. Additional options allow the applicant to identify the type of agency, vehicle class, and any specific plate designation needed (e.g., Volunteer Rescue, Civil Air Patrol, or National Guard). Applicants must affirm that the vehicle is covered by appropriate financial responsibility (insurance) and will be used strictly for governmental functions in compliance with Ohio Revised Code §4503.16.
