This form is issued by the Ohio Bureau of Motor Vehicles to request specialized license plates for vehicles that meet specific operational criteria under Ohio law. Applicants must indicate whether they are applying for a Senior Bus, Temporary Farm Bus, or Camp Bus plate. A Senior Bus plate is issued to buses used primarily for transporting people aged 65 and older, provided the vehicle seats 10 or more passengers. A Temporary Farm Bus plate is valid for 310 days per year and is intended solely for the transportation of agricultural workers directly involved in farming operations. A Camp Bus plate is available for school buses contracted by a board of education, allowing them to be used during summer vacation to transport children and authorized supervisors to and from nonprofit or charitable camps.
