Form MC 292 - DISCLOSURE OF EMPLOYMENT OR CONTRACT IN MICHIGAN SCHOOL SYSTEM. This form is used in Michigan courts when a person employed by, or regularly working under contract with, a school district, intermediate school district, public school academy, or nonpublic school has been convicted of, or entered a plea of guilty or no contest to, certain crimes listed in state law. As required by Michigan Compiled Law 380.1230d(2), the individual must immediately disclose their employment or contractual relationship with the school system to the court on this form. Copies of the completed form must also be provided to the prosecuting attorney, the Michigan Superintendent of Public Instruction, and the superintendent or chief administrator of the affected school system.
