UCS-6341 - APPLICATION FOR A TEMPORARY EXTREME RISK PROTECTION ORDER. This form is used to request a court order temporarily restricting an individual’s access to firearms, rifles, or shotguns when they are alleged to pose a serious risk of harm to themselves or others. This form may be filed by law enforcement officers, district attorneys, family or household members, licensed medical and mental health professionals, or certain school officials.
