SC-8 - DEFAULT AFFIDAVIT AND REQUEST FOR JUDGMENT. This form is used when a plaintiff asks the court to enter a default judgment after a defendant fails to respond within the required time. The form requires the plaintiff to certify service of the summons and complaint, confirm that the defendant is not a minor or incompetent, and state whether the defendant is in active military service as required under federal law. The filer must also attest to having personal knowledge or business records supporting every essential element of the claim and disclose any payments received since filing. SC-8 includes optional sections for requesting prejudgment interest and attorney’s fees, with detailed instructions for calculating interest.
