AMENDMENT OF FOREIGN REGISTRATION. This form is used by foreign business entities—such as foreign limited liability companies, partnerships, and limited partnerships—that are already registered to do business in Utah and need to update or amend their existing registration details. Entities may use this form to change their business name (either due to a name change in their home state or to comply with Utah’s naming regulations), update the jurisdiction in which they were originally formed, modify the stated purpose of the business, or change their registered agent and the agent’s Utah address. The form requires the entity to provide its Utah registration file number, current name on file, and the new or amended information. It must be signed by an authorized representative under penalty of perjury, certifying that the contents are true and correct.
