This form is used in the State of Utah to update or modify specific information related to a registered business trust with the Department of Commerce, Division of Corporations & Commercial Code. This form must be completed electronically, as handwritten submissions are not accepted. It allows authorized parties to amend the trust’s registration by changing the business purpose, updating the registered agent or their address, changing the principal business address, adding or removing individuals associated with the trust, or updating the addresses of the trust’s principals. All changes must be reviewed and affirmed by the trustees, who are required to sign the form to certify, under penalty of perjury, that the information provided is true and complete.
