Form MC 264 - ORDER OF DISQUALIFICATION/REASSIGNMENT. This form is used in the State of Michigan when a judge is unable to hear a case due to disqualification under Michigan Court Rule 2.003. This form allows the judge to state the reason for disqualification, which may include bias or prejudice, a serious risk of actual bias, appearance of impropriety, personal knowledge of disputed facts, prior involvement as an attorney in the matter, recent professional or personal associations with a party or attorney, financial interests, or close relationships with individuals connected to the case.
