MC 70a - REVIEW OF REQUEST FOR REASONABLE ACCOMMODATIONS AND RESPONSE. This form is used to request a review of a previously denied request for reasonable accommodations in a court setting under the Americans with Disabilities Act (ADA). It allows individuals—such as witnesses, jurors, attorneys, parties, or others involved in court proceedings—to provide detailed information about the type of court service, proceeding, or program they are attending, the dates accommodations are needed, the nature of their impairment, and the specific accommodations they require. Once submitted to the court’s ADA coordinator, the form initiates a formal review process, in which the court responds by either granting the request in full or in part, specifying the accommodations provided, or denying the request, citing reasons such as the applicant not qualifying as an individual with a disability, the request creating an undue burden, or the request fundamentally altering the nature of the court’s services.
