LIC-4016-FORM - LANDOWNER ATTESTATION: LOCAL ELIGIBILITY. This form is required as part of the licensing process for a commercial cannabis business to obtain a license in the City of Los Angeles. This form requires that the property owner or their agent: (1) acknowledge that the Applicant has the right to occupy the Business Premises identified on the Applicant’s license application for the purpose of conducting commercial cannabis activity; and (2) attest that the Business Premises location is not ineligible for licensure for certain reasons specified below. This form must be signed by the property owner, an agent of the property owner, or if an entity owns the property, an officer of that entity.
