Cannabis Delivery Vehicle Fleet Information {ENF-3004-FORM}

ENF-3004-FORM - CANNABIS DELIVERY VEHICLE FLEET INFORMATION. All vehicles used for delivery commercial cannabis activity must possess a Delivery Vehicle Placard. To obtain a Delivery Vehicle Placard, Licensees must submit this form to DCRCompliance@lacity.org, with the subject line “Cannabis Delivery Vehicle Fleet Information Form.” Please provide a list of all the delivery employees of the Licensee in Table 1 and a list of all of the vehicles used for the delivery of cannabis and cannabis goods in Table 2. DCR will issue a Delivery Vehicle Placard for each vehicle which shall be kept in the vehicle at all times and provided upon request to DCR, LAPD, or any other agencies. The Licensee shall notify DCR via email at DCRcompliance@lacity.org when a vehicle is added to or removed from the fleet by amending and resubmitting this form. www.FormsWorkflow.com
