FORM ER.4 – APPLICATION TO RELIEVE ESTATE FROM ADMINISTRATION (Short Form). This form is used in the Probate Court of Summit County, Ohio, to request that a decedent’s estate be settled without the formal process of full estate administration. It applies only when the total value of the estate is $4,500 or less and does not include real estate or stocks. The applicant must certify that the funeral bill has been paid, no creditors (including Medicaid, if applicable) will be negatively affected, and all asset values are accurate and easily verified. The form also requires listing all surviving relatives, such as the decedent’s spouse, children, or next of kin entitled to inherit under Ohio law.
