FORM ER.5 – ORDER RELIEVING ESTATE FROM ADMINISTRATION (SHORT FORM). This form is used by the Probate Court of Summit County, Ohio, to officially approve a request to bypass formal probate proceedings when the total value of the decedent’s estate does not exceed $4,500. The order is issued after the court reviews the application and confirms the decedent’s date of death and residence, verifies that funeral expenses have been paid, and determines that the estate's small value and lack of creditor issues justify summary administration.
